 Helpful Insurance Information before your visit to IUMG

In a managed care plan, a referral is your primary care physician's permission for you to see a specialist.

A referral explains:
  • How many times you may visit the specialist or have a treatment.
  • The time period in which you need to schedule these visits.

When you have a health problem, you need to call your primary care physician first. If you see a specialist without first calling your primary care physician, your health insurance may not pay for the appointment. You may have to pay the bill.

If you are not in a managed care plan and have preferred provider plan (PPO), you may see any specialist who is part of the plan. Check your physician list to be sure IU Medical Group physicians are preferred providers in your plan.

What is an authorization?

An authorization is permission from your health insurance plan for you to receive a certain service or treatment. For some medical services, you may need prior authorization in addition to a referral. Many PPOs also require prior authorization for certain services.

When I come for my appointment with an IU Medical Group specialist, what should I do?

  1. Have a referral from your PCP, if your insurance plan requires it. It may take several days to get a referral, so call ahead to ensure you have met your plans' requirements.

  2. Bring your insurance card with you to your appointment.

  3. Arrive at least 20 minutes prior to your appointment, if it is a first visit. This will allow time for you to complete the necessary forms for your medical records and your insurance company.

  4. Bring money for your co-payment, if you have one. A co-payment is a small amount - a part of the entire cost of your health care - that you pay when you receive care. If you are unsure of your co-payment, it may be noted on your insurance card, or you may want to contact your insurance company

  5. Check with the specialist's office staff before you leave to find out if you need another referral for future visits or authorizations for future procedures.

What if I come to IUMG for health care without a referral?

If your insurance plan requires a referral, you may want to reschedule your appointment until you have a referral from your primary care physician. If you see a specialist without a referral, you will have to sign an Insurance Waiver Form stating that you will be responsible for paying your bill.
